The Empty-Box Return: A Receiving Inspection Checklist for Ops Managers

Most return fraud is found or missed at one place: the receiving bench. A 60-second inspection step, applied the same way every time, catches more than any policy language.

What retailers are reporting

NRF's 2025 Retail Returns Landscape found that about 9% of returns are fraudulent. Among retailers that track it, 71% reported overstated quantities, 65% reported empty boxes or boxes of rocks, and 64% reported decoy returns such as counterfeit items. Those are the three patterns to design your checklist around.

Set the rule before the box arrives

Inspection works best when the customer knows it is routine. State in your return policy that refunds are issued after items are received and checked. Do not accuse anyone. Treat it as a standard step.

Decide in advance which returns skip inspection. Low-value items from customers with a clean history can be refunded on scan, which keeps your experience fast for the majority.

The receiving checklist

  1. Weigh the package before opening. Compare it with the expected weight of the item plus packaging. A mismatch is the fastest signal for an empty box.
  2. Photograph the sealed package, with the label visible.
  3. Open on camera or at a station with a fixed photo point. Keep it consistent.
  4. Match the SKU, size, and color to the return authorization.
  5. Check the quantity against the order. Overstated quantity is the most common pattern.
  6. Check authenticity markers on high-risk items: serial numbers, tags, stitching, packaging inserts.
  7. Check for wear, odor, and missing parts. Record the condition grade.
  8. Log the outcome, including time stamp and the staff member.
  9. Release the refund or flag a hold within a fixed time window, such as one business day.

Escalation steps

If the weight is off or the item does not match, stop. Do not refund yet, and do not send an accusatory email. Send a neutral message asking the customer to confirm what they shipped, attach your photos, and give a short deadline.

Escalate to a manager when the return is above a value threshold, when the account has multiple flagged returns, or when the item is high resale.

Protect the good customers

The goal is to catch the small share of bad returns without slowing the large majority of honest ones. Track your false flag rate, meaning the number of flagged returns that turned out to be legitimate. If it climbs, your thresholds are too tight.

Offer alternatives where a return makes no sense. For low-value items, a keep-the-item outcome or a partial refund skips the receiving step altogether. It is faster, cheaper, and removes the opening for return abuse.

Measure it

  • Time from delivery to inspection.
  • Share of returns flagged.
  • Share of flags that were confirmed.
  • Dollar value recovered or avoided.

Review monthly, and adjust one threshold at a time.
